Tixel 2 Thermomechanical Treatment for Meibomian Gland Dysfunction and Dry Eye
Tixel is a device that began life in aesthetic dermatology and has been adapted, with growing evidence, to treat the eyelids in meibomian gland dysfunction (MGD) and evaporative dry eye. It offers a different route to the same goal as thermal pulsation — restoring meibomian gland function — by delivering heat to the lids from the skin side.
How Tixel works
Tixel (Novoxel) is a non-ablative thermomechanical device. A tip made of medical-grade titanium is heated to around 400°C and, under precise control of protrusion distance and contact time (typically a few milliseconds), briefly touches the periocular skin. Because contact is so brief and fractional, it transfers thermal energy to warm the eyelid and the underlying meibomian glands without ablating or breaking the skin. The heat softens obstructed meibum and is thought to stimulate the glands and periocular tissue, improving lipid delivery to the tear film. Treatment is applied across the upper and lower lids, usually as a short course of sessions two weeks apart. TFOS DEWS III (2025) lists thermomechanical treatment among the emerging device-based options for MGD.
What the evidence shows
The most detailed prospective data come from Safir et al. (2022, Contact Lens and Anterior Eye), a pilot trial of 40 patients with dry eye due to MGD, each receiving three Tixel treatments two weeks apart. Comparing first and last visits, every measured parameter improved significantly:

Benefits persisted for at least a month after the final session, and no major side effects were reported. Subsequent prospective work (Shah et al., 2023) reproduced improvements in signs and symptoms, a 2025 observational extension (Sadri et al.) examined the durability of the effect, and further studies have explored Tixel's impact on tear lipid profile and on refraction and keratometry — the latter relevant when stabilising the ocular surface before cataract or refractive surgery.
An honest read of the evidence
The Tixel dry eye evidence base is promising but still early: the anchor studies are pilot and prospective cohorts rather than large randomised controlled trials, and the authors themselves call for double-blind RCTs. Its appeal is a quick, well-tolerated, skin-side route to lid warming that also addresses periocular skin — but it should be positioned as an evolving therapy, chosen on clinical judgement within a staged plan.
Device-based dry eye research at NZERC
A device at this stage of its evidence curve is precisely where a capable research site adds value.
