LipiFlow Vectored Thermal Pulsation for Meibomian Gland Dysfunction

Meibomian gland dysfunction (MGD) is the leading cause of evaporative dry eye, and the traditional home remedy — a warm flannel held to the lids — rarely delivers enough heat, for long enough, in the right place to clear obstructed glands. LipiFlow was designed to solve exactly that problem in a single in-office treatment.

How LipiFlow works

LipiFlow (Johnson & Johnson) delivers vectored thermal pulsation. A sterile activator sits over the eye: a heater warms the inner (palpebral conjunctival) surface of the eyelids to a therapeutic target of around 42.5°C — the temperature at which stagnant, thickened meibum liquefies — while a separate bladder applies graded, pulsatile pressure to the outer lid surface. Heating from behind the glands and expressing from in front, over a single ~12-minute treatment, physically evacuates the obstructed glands in a way that warm compresses and manual expression struggle to match. TFOS DEWS III (2025) places device-driven eyelid warming and thermal pulsation firmly within the evidence-based management of MGD.

What the evidence shows

Thermal pulsation is one of the better-studied dry eye devices. The American Academy of Ophthalmology reviewed the technology in a 2023 Ophthalmic Technology Assessment (Tao et al.) and concluded that thermal pulsation improves the signs and symptoms of MGD and dry eye, performing at least as well as conventional warm-compress therapy with a good safety profile. A 2025 Bayesian network meta-analysis (Chen et al.) pooling six LipiFlow randomised trials found meaningful improvements over control, summarised below.

A dedicated 2025 systematic review (Chen et al., Therapeutic Advances in Ophthalmology) supported LipiFlow as an effective standalone MGD treatment, and clinical work such as Mencucci et al. (2023) has shown value in reducing dry eye before and after cataract surgery, where an unstable tear film degrades biometry and visual outcomes. Earlier single-treatment studies (Blackie et al.) established that a single vectored thermal pulsation session can improve gland function for months.

An honest read of the evidence

The effect sizes over control are real but modest, the durability of a single treatment varies between patients, and — as the 2025 network meta-analysis stresses — there are still no head-to-head randomised trials directly comparing LipiFlow with intense pulsed light. In practice, LipiFlow is best understood as one precise, reproducible tool within a staged MGD plan, not a standalone cure.

Device-based dry eye research at NZERC

Reproducible, in-office thermal pulsation is exactly the kind of standardised, measurable intervention that clinical research is built on. The New Zealand Eye Research Centre (NZERC) runs its device-based dry eye therapies within a structured, measurement-driven ocular surface service — the infrastructure a clinical trial site requires. Research organisations, device manufacturers and pharmaceutical sponsors evaluating a New Zealand site for MGD, thermal-pulsation or evaporative dry eye studies are invited to partner with NZERC.
